Signed copies of these need to be turned in per each child participating. You can download & print your copies HERE, or you can come by and get copies from PCYFC.

• The original or certified copy of birth certificate is required for each participant. 

• A certified copy is NOT the same as a regular photocopy. Certified copies can be obtained from the register of deeds office in the county your child was born in and can be done online; they will mail it to you for around $10, but this could take a few weeks, so plan accordingly. All paperwork/requirements are due by August 1, 2025.

• YOU WILL GET THE BIRTH CERTIFICATE BACK AT THE END OF SEASON. 

• Any photo of your child will do; it can also be printed out on regular paper and can be wallet size.

• a photograph of each participant is part of what we turn in to WNC league to verify participants. See our FAQ page to learn more about the WNC league requirements. 

• A sports physical is NOT THE SAME as a well check. We need a current (given after Nov. 2024) sports physical where a doctor has specifically noted that the child is cleared to play sports. This is an insurance requirement.

• Sometimes the doctor office has their own sports physical forms, but you can also download and print one from HERE 

• Most urgent cares will do a sports physical for $25 cash.
